Does RU58841 Work? What the Evidence Actually Says

The internet is full of confident answers. The actual data is thinner than most people admit. Here is what the evidence supports, what it does not, and how to think about it honestly.

The honest short answer

RU58841 has a real mechanism (blocking DHT at the androgen receptor), controlled animal research showing that mechanism produces measurable hair regrowth in relevant models, and more than a decade of community use with mixed but broadly positive anecdotal reports. What it does not have, and has never had, is a completed human clinical trial. Anyone telling you it definitively works is going beyond what the evidence actually supports. Anyone telling you it definitively does not work is doing the same in the opposite direction.

What the animal research actually showed

The single most-cited RU58841 study is Uno et al., 1998, which used the stump-tailed macaque model of androgenetic alopecia (stump-tailed macaques develop pattern hair loss in a way that mirrors the human condition closely). Topical RU58841 produced statistically significant hair regrowth compared to vehicle. This is not a trivial study. It is a genuine controlled experiment in the most relevant animal model available.

The other important body of work is the mouse-grafted follicle studies: researchers took human scalp follicles from balding donors, grafted them onto testosterone-conditioned mice, and treated them topically. RU58841 improved outcomes over vehicle. This is not a mouse growing human hair out of nowhere; it is a controlled way to test the compound against actual human follicles under conditions similar to what happens on a human scalp.

What is missing

Human clinical trials. The kind of large, controlled, placebo-blinded studies that turn "promising" into "proven." RU58841 never went through this stage because Roussel-Uclaf shelved the compound in the 1990s for commercial reasons, not scientific ones. Nobody has funded the trials since. That is why every honest summary has to say research compound, not proven treatment.

What the community reports show

Fifteen years of use, thousands of self-reports, and a substantial body of photographic evidence. The dominant pattern in these reports is "modest to moderate improvement in maintenance and some regrowth over 6 to 12 months of consistent use." That is real signal. It is not evidence-grade data, because:

  • Selection bias: people who see results post; people who see nothing quietly move on.
  • Confounders: most users run RU alongside finasteride, minoxidil, or both. Isolating the RU contribution is impossible from anecdotes.
  • Placebo effects and photo variance: hair loss is slow and visual, lighting and angle change apparent results dramatically.

How to think about "does it work"

The evidence-grade answer is: the mechanism is real, the animal data is supportive, the human data is anecdotal but consistent, and no trial has confirmed it in humans. That is a fundamentally different answer from either "yes it works" or "no it doesn't." How you weigh those inputs is a decision for the individual researcher.
